Job Description:
Care Synergy Medical Group has an immediate opening for a
Physician based out of the Pathways Hospice location.Location: Ft.
Collins, COStatus: Full-timeSchedule: Monday-Friday, 32 hours per
week, on-callPay Range: $166,516 - $195,653 annually (32 hours per
week)Supplemental Pay: Based on position, schedule and/or
availability: Paid Mileage/Shift Diffs/StipendsMileage and Expense
Reimbursement: Sixty-two and a half cents per mile one of the

ProgramsREWARDING WORK YOU WILL DOProvides physician services
within the spectrum of hospice and palliative care. The Physician
guides, directs, and is a care consultant to the Outpatient,
Inpatient and Palliative departments staff, the patients attending
physician and other providers to assure standards, quality, and
excellence of care is provided. Fosters and maintains external
relationships between the hospice & palliative care programs and
other professionals and organizations engaged in health care
services.Provides physician services consistent with hospice and
palliative care philosophy. Consults with attending physicians and
palliative care/hospice staff as requested to address goals of
care, pain and symptom management for patients.In absence of
Medical Director, may be designated to assume responsibility for
the medical component of the patient care program.Reviews clinical
information upon patient referral to document disease diagnosis,
prognosis, therapeutic regiment and assessment of patients
health.Participates at Inter-Disciplinary Group/Team (IDG or IDT)
patient care meetings. Utilization review of patient care as
necessary.Determines clinical appropriateness of hospice and/or
palliative admissions, transfers palliative care to hospice, &
discharges hospice or palliative patients as needed.Assists the
President with program philosophy and service model for hospice &
palliative care.Follows hospice program policy when providing
supervision for cross coverage of physician service.Serves as
liaison with attending physicians, local hospitals, medical schools
and teaching hospitals. Provides support to attending and
consulting physicians, and oversees patient medical services. If
the attending physician is not available, the physician is
responsible for meeting the medical needs of a hospice
patient.Conducts home, Inpatient, or facility visits to hospice &
palliative patients, as needed.Participates in planning, developing
and evaluating the hospice & palliative care department services
within the agency as a whole, including but not limited to,
internal and external committees and input into development of
policy and procedures.Remains available for rotating schedule of
on-call assignments. Phone coverage after hours and weekends for
hospice and palliative consult service patients. Patient visits
after hours and weekends when necessary.WHAT WE ARE GOING TO LOVE
ABOUT YOU:Minimum Education: Graduate from approved school of
medicine.Minimum Experience: Three years experience as practicing
physician in hospice, internal medicine, family practice,
pediatrics, oncology or related field.Required License: Current
Colorado Physician License to practice medicine. Board certified
(or board eligible) in Internal Medicine or Family Practice or
Hospice & Palliative Medicine. Colorado state drivers license and
proof of insurance. Current CPR certification.PHYSICAL
REQUIREMENTS:Ability to lift minimum of 35 lbs.Care Synergy Medical

Colorados Front Range and the Regions Largest Home Health, Hospice,
and Palliative Care Network. Care Synergy provides mission support
services to better equip Care Synergy Medical Group to operate as a
distinct and independent organization while working together to
share best practices and serve more Coloradans along the Front
