Core Medical-Perm is seeking a LPN / LVN Home Health for a job
in Lakewood, Colorado. Job Description & Requirements -

Duration:Ongoing - 36 hours per week - Shift:12 hours - Employment
Type:Staff Home Health LPN - Days Territory:Lakewood / Centennial -
One year of previous LPN experience required - Requires knowledge
in nursing and person-centered care concepts - Ability to
demonstrate competency in the provision of individual care of
patients based on the patients' assessed age and developmental
needs. - HS GED - Current state LPN license for applicable state of
employment or current LPN license from a participating state in
National Compact Licensure (Colorado only). - Current
Cardiopulmonary Resuscitation (CPR) required; - Current Basic Life
Support (BLS) certification from American Heart Association,
preferred - Valid driver's license - LPN IV certification preferred
