UCHealth is seeking a dedicated and compassionate RN Acute Care Transition Specialist to join our Colorado-based nonprofit healthcare system. This role focuses on seamlessly transitioning patients between various levels of care within the hospital. The specialist will collaborate with a team of professionals to ensure that transitions are handled with the highest level of care and efficacy, adhering to all health regulations and standards. This position offers the opportunity to enhance patient outcomes by providing top-tier transitional care in a dynamic and supportive environment. Manage and coordinate the transition of patients between different care units or facilities. Collaborate with healthcare team members to develop care plans and strategies. Ensure compliance with healthcare standards, policies, and procedures during transitions. Educate patients and families about care plans and post-transition processes. Monitor patient conditions throughout transitions and adjust care plans as necessary. Strong clinical skills in a hospital setting Excellent communication and interpersonal skills Ability to work collaboratively in a team High attention to detail and organizational skills Medical Insurance Dental Insurance Vision Insurance Life Insurance 401k Current Registered Nurse (RN) license in the state of Colorado BLS/CPR certification
