HCA Healthcare seeks an experienced RN Case Manager who will take the lead in facilitating patient-centered care in a holistic, collaborative manner. This role is integral in ensuring that our patients, who range from chronic cases to critical complexities, receive optimal treatment pathways and that their continuum of care is seamlessly managed. Our RN Case Managers are essential in aligning patient needs with appropriate medical services through comprehensive assessment and planning, utilizing evidence-based practices. Assess patient needs and develop individualized care plans in collaboration with a multidisciplinary team. Facilitate timely coordination of quality healthcare services to meet an individual's health needs across the continuum of care. Advocate for patients and families to ensure that care is patient-centered and aligns with their values and needs. Monitor patient progress and outcomes, adjusting care plans as needed. Communicate effectively with all stakeholders, including patients, families, healthcare teams, and insurance providers. Ensure compliance with healthcare regulations and standards. Strong clinical judgment Excellent communication and interpersonal skills Proficient in healthcare management systems Ability to work collaboratively in a multidisciplinary team Medical Insurance Dental Insurance Vision Insurance Life Insurance 401k Current Registered Nurse (RN) license valid in the state of employment. Certification in Case Management (CCM) preferred.
